The penalty shot

1/4 A visitor has the puck past the home blue line with only the goalie to beat. A home player catches up and hooks from directly behind.

2/4 All five boxes ticked: control of the puck, past the blue line, nobody between them and the goalie, fouled from behind, a real chance lost. Penalty shot.

3/4 Everyone else goes to the benches. The puck sits on the center spot; the goalie stays in the crease until the shooter touches it.

4/4 The shooter must keep moving toward the goal and gets one shot. No rebounds.
A foul on a breakaway earns a penalty shot only if all five are true: the player had control of the puck, was over the defending team's blue line, had nobody between them and the goalie, was fouled from directly behind, and lost a real chance to score. A foul from the side is a penalty, not a penalty shot.
When the foul would normally be a minor, the fouled team may choose to take the minor (a power play) instead of the shot. If they take the shot, the offender is still charged on the scoresheet but does not sit. A major or worse is still served even if the shot scores.
The fouled player takes the shot. The goalie may not leave the crease until the shooter touches the puck. If anyone else interferes, the shot is taken again and that person gets a misconduct.
