1/3 Late in the third period, home has six skaters on the ice by mistake: too many players.

2/3 Normally that is a two-minute minor. In the last two minutes of regulation, or any time in overtime, a deliberate illegal change becomes a penalty shot for the other team.

3/3 Each team gets one one-minute time-out per game, called at a stoppage by a coach or captain. Coaches often save it for the final minutes.
Too many players on the ice is usually a minor. But in the last two minutes of regulation or at any time in overtime, a deliberate illegal substitution (too many players, or leaving the penalty bench early) is a penalty shot. If an extra player stops a breakaway, it is a penalty shot plus a misconduct.
Taking off your own helmet during play is a minor plus a misconduct; during a breakaway, in the last two minutes, or in overtime, it becomes a penalty shot plus a misconduct.
Each team has one time-out a game, one minute long, called at a normal stoppage before the puck drops. The other team may use that minute too, for example to warm up a goalie.
